Interprofessional Teams

In addition to Community Health Workers, we also have interprofessional teams of students with different expertise working together to care for participants of the Household-Centered Care program. They identify social risk factors and barriers for the community and work with the participants to find ways to overcome these challenges.

The teams are made up of UH students from the College of Nursing, the Graduate School of Social Work and the Tilman J. Fertitta Family College of Medicine and are overseen by licensed professionals who are also faculty members at UH. Participating in the Household-Centered Care Program helps educate our students about the “upstream” social determinants of health and provides them with skills to address community challenges impacting health as part of a team.
