Award Nomination Forms
Nominate and/or Recommend a staff employee for a President’s Excellence Award!
To nominate a staff member for an award, you should FIRST review the Award Guidelines AND Award FAQs. Then complete the Nomination Form below in its entirety. Next facilitate the submission of a minimum of 2 (but no more than 4) Recommendation Forms from the colleagues of your nominee in support of the nomination.
Colleagues who would like to support a nomination must use the Recommendation Form below to express their support of the nominee. (Recommendations submitted in letter format and/or as attachments will not be accepted.)
For an employee to be considered for an award, a Nomination From accompanied by at
least 2 Recommendation Forms must be submitted by Friday, July 25, 2025, by 5:00 PM.
