Award Nomination Forms
Nominate and/or Recommend a Staff Employee for the Next
President’s Excellence Award!
Nominations are currently closed and we will inform you once the nominations are open.
For future reference, if you would like to nominate a staff member, here's what you
should do:
- You should FIRST review the Award Guidelines AND Award FAQs.
- Once you have reviewed both you can then complete the Nomination Form below in its entirety.
- Next facilitate the submission of a minimum of 2 (but no more than 4) Recommendation Forms from the colleagues of your nominee in support of the nomination.
Colleagues who would like to support a nomination must use the Recommendation Form below to express their support of the nominee. (Recommendations submitted in letter format and/or as attachments will not be accepted.)
For an employee to be considered for an award, a Nomination Form accompanied by at
least 2 Recommendation Forms must be submitted.
