Report an EHS Concern, Incident, or Near Miss
University of Houston students, faculty, and staff may use the Report an EHS Concern, Incident, or Near Miss Form to report safety concerns, hazards, near misses, and incidents to Environmental Health & Safety (EHS). Reporting these issues helps the University identify and address potential risks and maintain a safe campus environment.
Emergencies
This form is not a substitute for emergency assistance.
If there is an immediate threat to health or safety, contact UHPD by calling 911 (via
Microsoft Teams or UHGo app) or 713-743-3333.
After emergency assistance has been obtained and the immediate situation is under control, complete any applicable injury or property damage reporting through the Risk Management.
Other Reporting Options
Please use the following University reporting channels for below concerns:
1. Safety/emergency equipment requiring repair
If a chemical fume hood, safety shower, or eyewash station is not functioning properly,
please submit a FIX-IT request to Facilities Services.
2. Fire incidents or fire safety concerns
Report fire-related concerns, including any fire that has already been extinguished,
to the UH Fire Marshal’s Office at fls@central.uh.edu or 713-743-5595.
3. For non-EHS concerns, or to remain anonymous

What should I report in this Form?
1. Safety Concerns
A condition, practice, or safety need that may require review, improvement, or assistance.
Examples include:

- Inadequate or missing safety related signage
- Expired or overdue chemical fume hood, safety shower or eyewash station inspection/certification
- An observed work practice that appears unsafe or inconsistent with established safety procedures
2. Hazards
A specific condition or situation with the potential to cause injury, illness, property
damage, or environmental harm if not controlled or corrected.
Examples include:

- Water or another slip hazard in a building walkway · Chemical waste found in an inappropriate disposal location
- Exposed electrical wiring in a work area
3. Near Misses
An unplanned event that could have resulted in injury, exposure, or property damage
at work but did not, either by chance or through timely intervention.
Examples include:

- A chemical spill that was contained before an exposure occurred
- A centrifuge spill involving biological material that did not result in exposure
- A slip or trip that nearly resulted in a fall or injury
4. Incidents
An unplanned event that results in an injury, illness, exposure, property damage or
other adverse consequences at work.
Examples include:

- A needlestick involving a biological or chemical hazard
- A chemical exposure requiring use of an eyewash or safety shower
- A slip, trip, or fall that results in an injury
EHS will review the information submitted and determine the most appropriate next steps. Some concerns may be addressed directly by EHS, while others may be referred to the appropriate University office, such as Facility Services, the Fire Marshal’s Office, Risk Management, Human Resources, for follow-up.
