The MVR Form, or Motor Vehicle Request form, must be completed by all individuals who wish to drive any of the EAS department owned vehicles.

• Please type the necessary information into the form, then print, sign and date. The signed form must then be returned to Marsha Braxton.
• Once vehicle clearance has been received, the EAS department business office will inform the appropriate parties.
