Charges, Insurance, and Payment Information

 

Insurance and Payment Policy

The UH Student Health Center accepts the UH System-endorsed Student Health Insurance Plan for most services. Commercial third-party insurance is accepted only for pharmacy and eligible laboratory services. The Student Health Center will file insurance claims only for the UH System-endorsed Student Health Insurance Plan. Patients with other insurance are responsible for submitting their own claims using the statement available through the Healthy Coog Patient Portal. All other services are billed directly to the patient at the self-pay rate. Patients are responsible for all charges not covered by their insurance.

For Patients Enrolled in the UH System-Endorsed Student Health Insurance Plan:

Once your coverage in the UH System-endorsed Student Health Insurance Plan is active for the current coverage period, please allow 3-5 business days for Academic Health Plans to update your insurance enrollment information in your Student Health Center records. The Student Health Center will file claims only for eligible charges under the UH System-endorsed Student Health Insurance Plan.

By signing this Agreement, you hereby understand and agree to the following:

  • It is your responsibility to renew your coverage in the UH System-endorsed Student Health Insurance Plan each coverage period.
  • You are responsible for the self-pay cost of any services provided at the Student Health Center should your coverage in the UH System-endorsed Student Health Insurance Plan lapse or become inactive.
  • You authorize the release of your medical records as necessary for the UH System-endorsed Student Health Insurance Plan billing (as applicable).

 

Fees and Cost Estimates

As a patient, you will be charged a fee for professional services, including but not limited to office visits. Additional fees may be charged for medications, laboratory services, procedures, supplies, injections, and other services provided, as determined by your treatment plan and the medical judgment of the clinic’s staff. Upon request, you may receive a cost estimate after your treatment plan or service has been determined by the medical staff.

Please Note:  All prices are subject to change without notice. Service costs may fluctuate based on the prices we pay for supplies and vendor services.

As applicable, the Student Health Center will transfer charges to university departments or a patient’s university financial account.

Payments for charges transferred to a student's university financial account are processed by the Office of Student Business Services. The patient is responsible for ensuring the Student Health Center has the patient’s current payment information. If a patient would like to change their payment information, they may do so through the Healthy Coog Patient Portal.

Laboratory Services and Payments

  • If you DO NOT have health insurance, or you choose NOT to use your third-party insurance for eligible laboratory services sent to Quest Diagnostics:  You will be charged at our self-pay rate for all services rendered. The amount owed will be transferred to your university financial account.
  • If you choose to use your commercial third-party insurance for eligible laboratory services sent to Quest Diagnostics: You will be charged at our self-pay rate for all services rendered except for the laboratory services sent to Quest Diagnostics. The amount owed will be transferred to your university financial account. Quest Diagnostics will file a claim with your insurance carrier for laboratory services sent to their facility.

By signing this consent form, you hereby acknowledge and agree to the following:

  • It is your responsibility to upload a copy of your health insurance card through the Healthy Coog Patient Portal and to notify the Student Health Center of any changes.
  • It is your responsibility—not the responsibility of the Student Health Center staff—to know the limitations of your health insurance plan and whether the plan will cover laboratory services provided by Quest Diagnostics.
  • You are financially responsible to Quest Diagnostics for all claims they submit to your insurance carrier, whether or not the services are a covered benefit.
  • A claim submission is not a guarantee of payment. You are responsible for confirming your benefits and eligibility directly with your insurance company.

Missed Appointments 

By signing this agreement, you hereby acknowledge your receipt and understanding of the UH Student Health Center Appointment Policy: No Shows, Late Arrivals, and Late Cancellations. Further, you hereby acknowledge and understand that a “No Show” fee will be posted to your university account if you fail to arrive at your scheduled appointment without advance notice of cancellation or late arrival. You hereby acknowledge and understand that you are financially responsible for any “No Show” charge that is posted to your account due to missing your appointment.


Athletics Billing with Authorization

The Student Health Center will bill the UH Athletics Department for services rendered to student-athletes upon receipt of an acceptable authorization from the University of Houston Sports Medicine Department. If you are a student-athlete, by signing this agreement, you hereby acknowledge and agree to the following:

  • You are responsible for requesting that the UH Athletes Department submit a payment authorization to the Student Health Center prior to your appointment.
  • You are fully responsible for all charges related to the services provided, regardless of the UH Athletics Department’s authorization.

If you are a student-athlete and are receiving services pursuant to a payment authorization by the UH Athletics Department, by signing this agreement, you hereby acknowledge and agree to the release of your medical records to the University of Houston Sports Medicine Staff for the purpose of billing and/or determination of sports eligibility.


UH Department Billing with Authorization

The Student Health Center will provide certain services to UH faculty, staff, or students on behalf of a UH department. Under such circumstances, the Student Health Center will bill the UH department for the limited services rendered so long as the Student Health Center has received an authorized UH department request for student health center services form. If you are a faculty member, staff member, or student receiving a limited service under this provision, by signing this agreement, you hereby acknowledge and understand the following:

  • You are fully responsible for all charges regardless of your department’s authorization.
  • It is your responsibility to inform the front desk at check-in that you have an authorization on file from your department. This requirement applies each time you visit the Student Health Center for services pursuant to an authorization from a UH department.

By signing this agreement, you hereby consent to the release of your medical records for the purpose of UH departmental billing, as applicable.


Fees Related to Eligibility for Services

All currently enrolled students at the University of Houston are eligible for all services at the Student Health Center. Term eligibility dates are based on the university’s Academic Calendar. Your college’s specific academic calendar does not alter the eligibility dates.

FALL TERM ELIGIBILITY FOR SERVICES:

Students enrolled in the Fall term are eligible for all services beginning the Monday prior to the first class day through the day prior to the first class day of the Spring term.

SPRING TERM ELIGIBILITY FOR SERVICES:

Students enrolled in the Spring term are eligible for all services beginning the Monday prior to the first class day of the Spring term through the day prior to the first class day of the Summer term.

SUMMER ELIGIBILITY FOR SERVICES AND SUMMER ELIGIBILITY EXTENSION FEE:

Students currently enrolled in any Summer sessions are eligible for all Student Health Center services during the Summer term beginning the first class day of the Regular Summer session through the day prior to the first class day of the Fall term.

The Summer Eligibility Extension Fee is not health insurance. Instead, the Summer Eligibility Fee is an access fee that allows individuals to use Student Health Center services during the summer through the day prior to the first class day of the upcoming Fall term if they meet the eligibility criteria. This fee is and cannot be prorated or credited. Therefore, the fee amounts remain the same even if paid after the summer eligibility period begins or scheduled appointments during the summer period are canceled.

  • Students who graduate in May can receive services at the Student Health Center for the subsequent summer period through the day prior to the first class day of the Fall term. A non-refundable Summer Eligibility Extension Fee will be posted to the student's university financial account at the time the first summer appointment is scheduled.
  • Students who are not currently enrolled in a Summer session, but who were enrolled for the previous Spring term may receive services at the Student Health Center subsequent summer period through the day prior to the first class day of the Fall term. A non-refundable Summer Eligibility Extension Fee will be posted to the student's university financial account at the time the first summer appointment is scheduled.
  • Students who are not currently enrolled in a Summer session or the previous Spring term, but are pre-registered for the upcoming Fall term, may receive services at the Student Health Center beginning July 15th (July 1st for College of Medicine students) through the day prior to the first class day of the Fall term. A non-refundable Summer Eligibility Extension Fee will be posted to the student's university financial account at the time the first summer appointment is scheduled.
