Eligibility Requirements

• Pharm.D. degree and PGY-1 residency training (or equivalent experience)
• Pharmacy licensure within state of Texas
• Curriculum vitae and letter of intent
• Three letters of reference (Completed PhorCAS recommendation forms [PDF] are also acceptable)
• College of Pharmacy transcript
