Anyone can use this form to report incidents of discrimination or sexual misconduct.

We strongly encourage our community members to notify us if they feel that they or another member of the UH community has been discriminated against on the basis of a protected class [race, color, national origin, religion, age (40+), sex (including pregnancy), sexual orientation, gender identity, gender expression, disability, genetic information, or veteran status].
