Department Address or Name Change Request Form

NOTE: This form should only be used to change mail delivery for an ENTIRE Department that is moving. If a department is split between different locations on campus, there will be only one centralized address and one mail drop location.

Individual addresses can not be changed using this form. The form can also be used for a departmental name change.

Submit this form to have your department mail changed as a result of a Department move, or department name change.
