In cases of immediate danger or threats to safety, please call 911 or contact the UH Police Department at (713) 743-3333 without delay.

When To Report

  • Student Care Referral

    The University of Houston's Care Team, led by the Dean of Students, offers collaborative support for students facing challenges. Comprising experts with access to campus resources, the team assesses concerns and develops plans to promote wellbeing and safety within the UH community. Faculty, staff, students, and community members are encouraged to refer students experiencing physical, emotional, academic, or psychological difficulties affecting their overall well-being and student success.

  • Student Conduct Referral

    Student Conduct reports should be submitted when an alleged code of conduct violation may have occurred. Examples include, but are not limited to: misuse of drugs and/or alcohol, disruptive behavior, hazing, falsification of documents/records, mental and/or physical violence, theft, etc.

    Reports submitted anonymously create a credibility complication. Anonymous reports may not result in action due to the inability to investigate without a credible reporting source.

  • Title IX/ Sexual Misconduct/ Discrimination Reporting Form

    "Title IX" is part of the federal Education Amendments Act of 1972, which prohibits sex-based discrimination in federally funded educational programs, including at the University of Houston. Sexual harassment, including sexual violence, is a form of sex discrimination. Use this form to report violations of the University's non-discrimination and sexual misconduct policies (including dating violence, domestic violence, stalking, sexual assault and other forms of unwanted sexual contact).

    Employees: Use this report form to complete sexual misconduct mandatory employee reports required under Title IX and the Texas Education Code.
