The Coogs Report is the University of Houston’s centralized system for submitting concerns about student well-being, safety, and behavior, through which the UH Care Team collaborates with campus partners to assess needs, provide outreach, and connect students to appropriate resources.

When To Report

  • Student Care Referral

    The University of Houston’s Care Team provides a collaborative approach to supporting students facing challenges. The team assesses reported concerns and develops coordinated plans to promote student well-being and safety within the UH community. Faculty, staff, students, and community members are encouraged to submit referrals for students experiencing physical, emotional, academic, or psychological difficulties that may impact their overall success.

    The Care Team provides support to currently enrolled students and individuals who have been enrolled within the past 13 months. Students who are not enrolled in a subsequent semester remain eligible for Care Team support during this period. Individuals who have not been enrolled for 13 consecutive months are no longer considered active students and are not eligible for ongoing Care Team services unless they are readmitted to the university.

  • Student Conduct Referral

    Student Conduct reports should be submitted when an alleged code of conduct violation may have occurred. Examples include, but are not limited to: misuse of drugs and/or alcohol, disruptive behavior, hazing, falsification of documents/records, mental and/or physical violence, theft, etc.

    Reports submitted anonymously create a credibility complication. Anonymous reports may not result in action due to the inability to investigate without a credible reporting source.

  • Title IX/ Sexual Misconduct/ Discrimination Reporting Form

    "Title IX" is part of the federal Education Amendments Act of 1972, which prohibits sex-based discrimination in federally funded educational programs, including at the University of Houston. Sexual harassment, including sexual violence, is a form of sex discrimination. Use this form to report violations of the University's non-discrimination and sexual misconduct policies (including dating violence, domestic violence, stalking, sexual assault and other forms of unwanted sexual contact).

    Employees: Use this report form to complete sexual misconduct mandatory employee reports required under Title IX and the Texas Education Code.
