The Student Outreach and Support Services team is committed to your well-being and success. It is our honor to reach out and work with you as you transition back to campus, after a hospitalization, to ensure continued care and support.

Student Support Process
- A Student Care and Concern Report is submitted.
- The care team reviews the case and assigns it to a case manager (member of SOS Team).
- The case manager (member of SOS Team) may reach out to the reporting party for additional details.
- Outreach is initiated with the student and other relevant individuals.
- The member of the SOS Team meets with the student to evaluate their primary needs and overall well-being.
- Member of SOS Team identifies appropriate resources.
- An action plan is collaboratively developed by member of the SOS Team and the student.
- Member of SOS Team conducts follows up and reassesses the case as needed.
- Ongoing access to support and services remains ensured for the student.
