To find a Health Care Network provider:

1. Go to www.careworks.com
2. Click on “Find A Provider” (Located in the top right corner of home page)
3. Choose "Careworks TX HCN" from menu 
4. Choose “Address Search”
5. Enter TX for state and enter the zip code
6. Enter distance in miles 

For Provider Type, choose “Treating Providers” then click on "Find Providers" button at the bottom.
