Each semester, all Language and Culture Center (LCC) students in F and J statuses will be automatically enrolled in and charged for the UHS Student Health Insurance Plan. Please follow the steps below to create an insurance account.

Step 1. Visit www.uhcsr.com/CreateAccount and click “Create Account.”

Step 2. Enter your first name, last name, date of birth, and 7-digit myUH ID. Click “Next.”

Step 3. Create a user name and a password. The password must contact all of the following:

• an uppercase character
• a lowercase character
• a numeric character (0-9)
• a special character (e.g. ?, %, *, $, etc.).

Step 4. Log into your account and:

• view insurance ID card
• find a doctor
• view coverage details
• check claim status and Explanations of Benefits

Step 5. Search UHCSR on App Store or Google Play and download the mobile app.
