Fiscal Year 2019 - 2nd Quarter MAPPs in Executive Review

The following MAPPs have been recommended for review by University of Houston Subject Matter Experts. To review the proposed changes, please select each policy name to view the linked document and send your comments, if any, to uhpolicies@uh.edu by February 4, 2019 (Monday).

The column titled "Proposed Changes" briefly explains the updates that have been submitted.

MAPP 03.02.02 - Delivery Services Policy
(New MAPP Title)
Changed the policy name from "Non-Permit Mailing" to "Delivery Services Policy." The entire procedure was rewritten to emphasize mailing services, including documentation on residence halls and mail, suspicious/lost/stolen packages, and the Departmental Mailing Card (DMC). Changed the review period from every three years to every five years on or before August 31. Removed Addendum A.
Updated were made by University Services and UH Printing and Postal Services.

MAPP 04.05.02 - Cougar Cards Changed area in Section 04.05 from Auxiliary Services to Auxiliary Services/University Services. Removed Section III.I on registered students paying mandatory Cougar Card Fee. Changed Cougar Card Online Service to My Photo in Section IV.A. Deleted Section IV.D on mailing out Cougar Cards. Added Birth Certificate or Social Security Card to definition of government-issued identification in Section IV.E. Removed $10 charge for lost/stolen cards in Section V.A. Revised Section V.B to be Nonfunctioning/ Worn Cards, and the requirements for replacement. Also gave the requirements for replacement for name/classification change in Section V.C. Removed Section VI.B on replacement fee of $10.00. Changed photo identification to government-issued identification in Section VI.D. Removed Sections VII.A, B, and C on financial liability for lost and stolen cards. Deleted sentence on cardholders receiving an electronic receipt at time of purchase in Section VIII. Changed review period from every three years to every five years on or before March 1. Changed responsible party to Associate Vice President for Administration. Updated links as indicated.

MAPP 04.05.03 - Preferred Names on Cougar Cards
(New MAPP)
The Cougar Card Office recognizes that faculty, staff, and students may use names other than their legal names to identify themselves. The use of preferred names is supported by the University's systems, including the campus card management system. This document establishes guidelines for the use of preferred names on the University of Houston's official identification card, the Cougar Card. This policy applies to university students, faculty, and staff.
This MAPP was originated by University Services and the UH Cougar Card Office.

MAPP 06.01.02 - Continuity of Operations Planning
(New MAPP Title)
Changed title of document from "Business Continuity Planning" to "Continuity of Operations Planning" to reflect current operating requirements. Changed all references to Business Continuity Planning to Continuation of Operations Planning (COOP) throughout document. Added Texas Labor Code, Section 413.054 to Section 1. Added information to Section II that the UH Office of Emergency Management will coordinate the review process annually, and materials required for development of COOP are confidential and exempt from disclosure under Texas Government Code, Section 552. Removed business impact analysis from Section 1,
Section III.D and Section IV.A.9. Changed review period from every three years to every five years on or before September 1. Added Texas Continuity Policy, Texas Labor Code, Section 412.054, and Texas Government Code, Chapter 552, Section 156 to Section VIII, References.
Updated were made by the Office of Emergency Management.

MAPP 06.03.04 - Food
Sanitation Program

(New MAPP)
The purpose of the Food Service Sanitation Program is to protect the University of Houston faculty, staff, students, and visitors by providing safe, and sanitary, food establishments on campus with implementation of the Hazard Analysis Critical Control Point (HACCP) program. The Food Service Sanitation Program shall apply to all food establishments on the University of Houston campus. These approved food establishments shall include, but not be limited to residential cafeterias, delicatessens, caterers, food trucks, temporary food service establishments, farmers' markets, and any other establishment where food and beverages are prepared or served to the UH community, whether for a charge or no charge.
This MAPP was originated by the Office of Environmental Health and Life Safety (EHLS) and the Office of Campus Safety.

MAPP 06.04.01 - Clinical and Research Use of Controlled Substances and Dangerous
Drugs
Updated links and titles as applicable. In Section III, faculty or staff members who work with controlled substances must obtain and keep current DEA, DPS, PMP and all other registrations unless exempted by law per the Texas Prescription Monitoring Program (PMP). Changed title to Environmental Health and Life Safety in Section III.H. Removed Section VI, Index Terms. Changed review period from every three years to every five years on or before August 1.
Updates were completed by the Division of Research (Animal Care Operations).

MAPP 09.03.02 - Campus Facilities Planning Committee (New MAPP) This document defines the roles and responsibilities of the Campus Facilities Planning Committee (CFPC). This committee is charged with reviewing and approving physical changes that affect the exterior planning, public spaces, architectural character and aesthetics of the University.
This MAPP was originated by the Office of Facilities/Construction Management.

MAPP 11.03.01 - Tuition, Fees and Charges Corrected links and titles as applicable. Removed Sections IV.B.3 and VI.B.3 on International Education Fees. Added information to Section X on year-end equity balances from voluntary fees (Fund 2170) not exceeding 25% of total revenue collected during the fiscal year. Changed review period from every three years to every five years on or before August 31.
Updates were completed by the Office of Finance (Funding and Budgets).

MAPP 12.01.01 - Scholarship Administration Updated links and titles as applicable. Changed "required registration fees" to "mandatory fees" in Section II.A. Changed Associate Vice President for Graduate and Professional Studies to the Vice Provost and Dean for the UH Graduate School in Section II.B; also replaced MAPP 05.03.02 with
MAPP 11.03.03. Added link to 20 U. S. C. 1087LL for Cost of Attendance definition in Section IV.D.2. Changed Office of Graduate and Professional Studies to UH Graduate School in Section V, first paragraph. Changed Section VI from "Stewardship Requirements for Endowed and Other Philanthropic Scholarships" to "Gift Compliance Requirements for Endowed and Other Philanthropic Scholarships." Changed Office of Stewardship to Office of Gift Compliance for Sections VI.A and VI.B. Changed review period from every three years to every five years on or before August 31. Added Senior Vice President for Academic Affairs and Provost and Vice President for Student Affairs and Enrollment Services to Responsible Parties
(Section VIII). Added Section X, References.

MAPP 13.01.02 - Conduct Assessment and Response
Team (CART)
Removed Director of Counseling and Psychological Services (CAPS), the Psychiatrist from the University of Houston Health Center, and the Captain from University of Houston Police Department (UHPD) in Section III.A. Added Assistant Director of Student Housing and Residential Life, and representatives from CAPS, Equal Opportunity Services (EOS), and UHPD to Section III.A. Changed review period from every three years to every five years on or before June 1.
Updates were completed by the Office of the Dean of Students (Division of Student Affairs and Enrollment Services).
