MAPPs in Campus Review, Quarter 2, Fiscal Year 2019

The following MAPPs have been recommended for review by University of Houston Subject Matter Experts (SMEs). To review the proposed changes, please select each policy name to view the linked document and send your comments, if any, to uhpolicies@uh.edu by December 21, 2018 (Friday).

The column titled "Proposed Changes" briefly explains the updates that have been submitted by the SME for Campus Review.

The entire procedure was rewritten to emphasize mailing services, including documentation on residence halls and mail, suspicious/lost/stolen packages, and the Departmental Mailing Card (DMC). MAPP 04.05.02 - Cougar Cards

Changed area in Section 04.05 from Auxiliary Services to Auxiliary Services/University Services. Removed Section III.I on registered students paying mandatory Cougar Card Fee. Changed Cougar Card Online Service to My Photo in
Section IV.A. Deleted Section IV.D on mailing out Cougar Cards. Added Birth Certificate or Social Security Card to definition of government-issued identification in Section IV.E. Removed $10 charge for lost/stolen cards in Section V.A. Revised Section V.B to be Nonfunctioning/Worn Cards, and the requirements for replacement. Also gave the requirements for replacement for name/classification change in Section V.C. Removed Section VI.B on replacement fee of $10.00. Changed photo identification to government-issued identification in Section VI.D. Removed Sections VII.A, B, and C on financial liability for lost and stolen cards. Deleted sentence on cardholders receiving an electronic receipt at time of purchase in Section VIII. Changed review period from every three years to every five years on or before March 1. Changed responsible party to Associate Vice President for Administration. Updated links as indicated.

MAPP 04.05.03 - Preferred Names on Cougar Cards

The Cougar Card Office recognizes that faculty, staff, and students may use names other than their legal names to identify themselves. The use of preferred names is supported by the University's systems, including the campus card management system. This document establishes guidelines for the use of preferred names on the University of Houston's official identification card, the Cougar Card. This policy applies to university students, faculty, and staff. (New MAPP)

MAPP 06.01.02 - Continuity of Operations Planning

MAPP 06.03.04 - Food Sanitation Program

The purpose of the Food Service Sanitation Program is to protect the University of Houston faculty, staff, students, and visitors by providing safe, and sanitary, food establishments on campus with implementation of the Hazard Analysis Critical Control Point (HACCP) program. The Food Service Sanitation Program shall apply to all food establishments on the University of Houston campus. These approved food establishments shall include, but not be limited to residential cafeterias, delicatessens, caterers, food trucks, temporary food service establishments, farmers' markets, and any other establishment where food and beverages are prepared or served to the UH community, whether for a charge or no charge. MAPP 06.04.01 - Clinical and Research Use of Controlled Substances and Dangerous Drugs

In Section III, faculty or staff members who work with controlled substances must obtain and keep current DEA, DPS, PMP and all other registrations unless exempted by law per the Texas Prescription Monitoring Program (PMP). MAPP 09.03.02 - Campus Facilities Planning Committee

This document defines the roles and responsibilities of the Campus Facilities Planning Committee (CFPC). This committee is charged with reviewing and approving physical changes that affect the exterior planning, public spaces, architectural character and aesthetics of the University. (New MAPP)

MAPP 11.03.01 - Tuition, Fees and Charges
