MAPPs in Interim Review and Approval, Quarter 1, Fiscal Year 2013

The following MAPPs have been recommended for interim review and approval by University of Houston Subject Matter Experts (SME). To review the proposed changes, please select the policy name to view the linked document.

MAPP 02.02.01 - Family and Medical Leave
(Interim 2)

This MAPP is being submitted for Interim Approval based on new procedure for submitting FML forms and Physician's Approval. MAPP 02.02.02 was originally in Q4, FY 2012 review cycle, but was removed to communicate this new process.
Added note to Section III.E regarding employees with student status as a condition of employment are not considered eligible for this program. In Section IV.J, added information on unpaid FML on the first workday of the month and rate of accrual for vacation leave. In Section V.C, added the procedure for notification the employee's supervisor of FML, and e-mailing the eligibility notice to the employee. In
Section VI, changed the process for submitting the Application for Family and Medical Leave to Human Resources instead of the employee's supervisor. Human Resources will submit the ePAR per documentation in Section VI.G. Added the Employee's Retirement System of Texas (ERS) to Section VII.B. In Section VII.D.4, the time period to submit recertification to Human Resources changed from 15 calendar days to 12 calendar days. Added
Section VIII.B for employing department to complete an ePAR when the employee returns to active status after FML. Added Section XI, References.

MAPP 02.02.02 - Parental Leave (Interim)

This MAPP is being submitted for Interim Approval based on new procedure for submitting FML forms and Physician's Approval. MAPP 02.02.02 was originally in Q4, FY 2012 review cycle, but was removed to communicate this new process.
Applied revised MAPP template and added new Revision Log. Added note for Section III.B regarding employees with student status as condition of employment are not eligible for this program. In Section IV.A, the employee must use accrued compensatory time, instead of unpaid parental leave. In Section V.A, changed the location of obtaining the FML forms to the HR Service Center. In Section V.B, added the responsibility for the employee to submit physician's certification and FML form to the HR Benefits Section. In Section VI.B, changed the insurance coverage contact to the Employee's Retirement System of Texas (ERS). Added Section XII, References.

MAPP 06.05.03 - Motor Vehicle Record Evaluation (Interim)
Office of Risk Management
This MAPP is being submitted for Interim Approval based on new procedure for annual Motor Vehicle Record (MVR) checks.
Removed information on personal vehicles used for university business in Section II.A. MAPP 06.05.03 now emphasizes vehicles owned or leased by the university, and the requirement for employees to fill out an MVR for driving vehicles which are owned or leased by the university. In Section II.C, the MVR should be filled out by employees prior to extending the offer of employment in cases where driving is an essential job duty. In Section III.A.1, the Driving While Intoxicated (DWI) or Driving Under the Influence (DUI) conviction requirement changed from five years to three years.
