Use proper channels when sending workers’ compensation forms

The Risk Management department currently processes workers’ compensation claims per UH policy MAPP 02.02.06 (Workers’ Compensation).

The Texas Business and Commerce Code defines someone’s driver’s license number and Social Security number in combination with the individual’s first name or first initial and last name as sensitive data.

Per UH policy MAPP 10.05.03 (Data Classification and Protection), sensitive data such as that defined in the Texas Business and Commerce Code, is Level 1 data and must not be transmitted by email unless the message is encrypted. To receive a digital certificate necessary to send encrypted email, contact UIT Security Manager Brian Walker at bmw@uh.edu.

Please ensure workers’ compensation forms are not sent by email unless the message is encrypted. Sending non-encrypted messages with Level 1 data is a violation of UH policy as well as state law and will not be accepted for processing.

If applicable, workers’ compensation forms should be completed in their entirety and submitted preferably by secured fax to 713-743-1501 or sent via personal and confidential inter-office mail, or delivered in person at 4211 Elgin St., Room 183, Houston, TX 77204. Risk Management does not have a mail code, so address inter-office communication as “Attention: Risk Management.”
