Vending Machine Refund

What must I do get a refund for money lost in a vending machine?

Step 1- Call University Services and report the problem with the machine. Dial
713-743-5723 and let the representative know the following:

-what type of machine took the money (beverage, snack, fresh food, frozen food, coffee or ice cream
-what location (building name)
-a description of what happened
-amount of money lost

Note: You can also file your report by providing the information mentioned above via email at universityservices@uh.edu or by completing the on-line form available at www.uh.edu/busservices (click on vending).

Step 2 � Complete the Vending Refund Request Form and click here for the refund request form then submit it to one of the three refund locations on campus.

Vending Refund Locations
-Cashier�s Office in the Welcome Center
-Business Office in the University Eye Institute
-Purchasing Department in University Business Park

Take proper ID (valid Cougar 1 Card or driver�s license). It is required to get your refund.

After a quick review of the information on the completed form, you will receive the refund right away.
