  • Annual Checkup

  • Annual checkups are regular wellness visits where your doctor will go over your overall health focusing on prevention.
  • Includes but not limited to:
      • Annual Physicals
      • Well Woman Exams
        (Member is responsible for all costs associated with laboratory testing and specimen analysis)
      • Well Child Checkups
        (Does not include vaccines)
  • Sick Visits

  • Sick visits are consultations with your doctor and care team when you are feeling unwell or experiencing unusual symptoms. You will receive a health evaluation and provided appropriate care recommendations.
  • Includes but not limited to:
    • Fever
    • Cough
    • Rash
    • Urinary Tract Infection
    • Headache
  • Disease Management

  • Manage your chronic medical conditions with a knowledgeable medical team who knows you, understands your challenges and works with you to come up with a care plan to improve your health and wellbeing.
  • Includes but not limited to:
    • Hypertension
    • Diabetes
    • Heart Failure
    • Depression
    • Renal Disease (No dialysis)
    • Musculoskeletal conditions
    • Pediatric Chronic Conditions (asthma, obesity, ADHD)
  • Women's Health

  • We provide essential services women need to maintain their health and live healthy lives.
  • Includes but not limited to:
    • Well Woman Exams
    • Family Planning Counseling
    • Prescription Birth Control Options
    • Prenatal Lactation Counseling
    • Postpartum Care ( this is covered by insurance or CHIP)
    • Breastfeeding support
  • Men's Health

  • We provide men’s health services to help members be healthy and enjoy life.
  • Includes but not limited to:
    • Sexual health
    • Hair loss
    • Low energy
    • Prostate problems
  • School/Sports/Camp Physicals

  • This an exam performed usually annual to evaluate your or your child’s fitness condition to enable participation in different activities.
  • Includes but not limited to:
    • School, Camp and Sports physicals for children (0-18)
    • Sports/Employment physicals for adults (18 +)
  • Procedures

  • Our team is experienced in various procedures.
  • Includes but not limited to:
    • Rapid Strep and flu – To check if you have streptococcal pharyngitis or influenza
    • Laceration Repair (minor) – To place sutures on minor cuts
    • Casts/Splints – To place a splint
    • Abscess – To drain
    • Simple skin cyst removal
    • Injections
  • Health Promotion

Pharmacy Services

Tilman J. Fertitta Family College of Medicine partners with the pharmacy at San José Clinic to provide pharmacy dispensing services for UH Health Direct Primary Care.


Eligibility

Members referred must be uninsured and fall under the 300%  Federal Poverty Line (FPL).
Eligibility process will be done at the UH Health Direct Primary Care.
Members will be notified when their prescription is ready and will be asked to make a $10 contribution  for each visit.

    Patient Assistance Program

    A patient assistance program is available to members who qualify based on the criteria set forth by the pharmaceutical companies to help with their health care costs. Applications will be completed onsite at San José Clinic upon receipt of required documents.

Diagnostic and Specialty Services

Access to discounted specialty services and high-end diagnostics for our members will be provided through our partnership with Memorial Hermann Southwest Hospital.

Laboratory Draws and In-House Tests

Includes but not limited to:
  • Blood glucose test
  • Pregnancy test
  • H1aC POC test
  • Rapid strep test
  • Rapid flu test

In-House Diagnostics

Incldes but not limited to:
  • EKG
  • Spirometry
  • Screening audiometry

Telehealth Services


As a member of UH Health Direct Primary Care, you can talk to your doctor or nurse from the comfort of your home using your phone, tablet or computer to connect with your care team. Our telehealth services include chat and video communications.
