ERS Open Enrollment 2011

Annual Enrollment for Plan Year 2012 is July 11 through August 5. During this time, you can change your benefits and enroll eligible dependents in coverage, effective September 1. You will receive more information in the mail from ERS.

Have you moved or changed your phone number or email address?
Update your new contact information in your ERS account by June 3, 2011 at 5:30 p.m. so you can receive benefits information from ERS this summer:

� Log in to your account from the ERS website. Click the Sign In button at the top. You may need to register your account.
� Click the appropriate link under My Personal Information to make your changes.

UH Benefits Fair

The UH Benefits Fair is scheduled July 13, 2011, from 10:00 a.m. to 4:00 p.m. in the Houston Room in the University Center. Our theme for this year is ��Navigating Through the World of Benefits.� Come by to visit with benefit representatives, UH department representatives, register for prizes, and get lots of freebies including the 2011 Benefits Fair T-Shirt.

New Optional Benefits

UHS is proud to partner with Aflac and MetLife Resources to offer optional benefits to benefits-eligible employees effective September 1, 2011. Sign up for the new benefits between now and August 5, 2011.
Aflac is offering these pre-tax benefits:
   Cancer Policy
   Specified Health Event Policy  
   Accident Policy
   Hospital Policy

MetLife Resources is sponsoring a Trustmark product, a life insurance product that offers two benefits in one policy:
   Permanent life insurance and living benefits for long-term care

Changes for Plan Year 2012

On May 24, the ERS Board of Trustees approved health insurance carriers and preliminary premium rates and optional insurance rates for Plan Year 2012. Premium rate increases, effective September 1, were needed to address the insurance funding gap. ERS will send more information on Plan Year 2012 benefits prior to this summer�s Annual Enrollment, July 11 - August 5. During the meeting, the Board:

� Increased the premium rates for HealthSelectSM of Texas and the Community First and Scott & White Health Plan HMOs by an average of 8%;
� Kept the Dental DHMO premiums at the same rate as last year;
� Increased premiums slightly for the State of Texas Dental Choice PlanSM; and
� Kept premiums for optional insurance�Optional Term Life, Dependent Term Life, Short- and Long-term Disability Insurance, and Voluntary Accidental Death and Dismemberment�at the same rate as last year.


Updating Your Personal Information in P.A.S.S.

With Benefits open enrollment fast approaching, update your home phone number, add cell phone numbers, emergency contacts and update home address information in P.A.S.S. It�s fast and simple. Log into https://my.uh.edu, enter your employee ID and password:
   Click on Personal Information,
   Click on Personal Information Summary

Use the scroll bar on the right side of the screen to scroll through your personal information, click on the yellow change button to update your information.

Update your work address, update the building code, room number and your office (main) phone number, click on the yellow change button to update your office information.

Can�t remember your employee ID or password? Click on the link on the page to request your employee ID or request a new password.
