UHS Individual Setup Form (MS Excel | Adobe(pdf))
This form must be completed by the following individuals in order to receive voucher payments:
   • UHS employees
   • UHS students
   • UHS prospective employees
   • UHS regents
   • Refund recipients (form can be signed by UHS department administrator)

UHS Vendor Setup Form (Interactive pdf | Adobe(pdf))
This form (or the W-9 Form) must be completed by the following individuals or businesses to receive voucher payments:
   • Sole proprietorships
   • Partnerships
   • Corporations
   • Government entities
   • Universities
   • Independent contractors
   • Lecturers/artists
   • Research study participants

Direct Deposit Authorization Form
Complete this form if you would like to be paid via direct deposit. Fax the completed form to 713-743-0521. If you are not setup as a vendor in our vendor file, you would also need to complete the UHS Individual Setup Form or UHS Vendor Setup Form.
