5 STEPS TO STUDY ABROAD!

Special Programs

STEP 1.

• Schedule an apppointment with Study Abroad Advisor or Director.
• Fill out and submit your study abroad on-line application form to your OISP Advisor.
• Research availability of study abroad programs with Kukulcan Educational Spanish Community (Fact Sheet)and Sol Education Abroad (Fact Sheet 1) (Fact Sheet 2) (Fact Sheet 3).

STEP 2.

• Log in to complete your Course Equivalency Form on-line and print it out.
• Get your Course Equivalency Form signed by Advisor/Faculty and Dean. Submit a copy to your OISP Advisor.
• The $100 non refundable fee will be posted to your UH account. Make payment to the Student Financial Services after the $100 fee has been posted.
• Apply with Kukulcan Educational Spanish Community or Sol Education Abroad and submit the acceptance letter to the OISP Office.

STEP 3.

Submit a copy of the following forms to OISP Advisor.

• READ ONLY : Release and Waiver of Liability Agreement
• Sign, notarize and submit the following :
  • Consent to Release Information
  • Consent for travelers under the age of 18 , if applicable.
• Sign and submit the Affidavit .
• Sign and submit the Health Disclosure Forms.
• Get your ISIC Card from UC Business Office (Cost: $ 25).
• Fill out and submit your State Registration Form for your trip to your OISP Advisor.
• Submit a copy of your passport page.
• Submit a copy of your visa, if applicable.
• Submit a photo for your file to the OISP advisor.

STEP 4.

• Meet with the Director Ms Parul Fernandes by scheduling an appointment with the Secretary at 713-743-9167 to sign off for registration after you complete your file.
• Attend Pre-departure Orientation (Mandatory).

STEP 5. Re-entry

• Upon your return, schedule a re-entry appointment with your advisor, Maria Saldana, at 713-743-9206 or mjsaldana@uh.edu. Bring your one page essay and your transcript when you meet with Ms. Saldana.
• Ask for the evaluation form to be sent to you electronically.
• Schedule an appointment with the Secretary to meet with Ms. Parul Fernandes at 713-743-9167.
