Nomination for the
PROVOST FACULTY ADVISING AWARD
For Excellence in Undergraduate Academic Advising
Spring 2010

The University Advising Award Committee will select one faculty advisor to receive the Provost Faculty Advising Award. The recipient will receive a plaque and monetary award of $1000.

Eligibility: Any faculty member whose responsibility is to provide academic advising services to undergraduate students at the University of Houston may be nominated for this award. Self-nominations are also accepted. Previous recipients are eligible for nomination in the sixth academic year following a prior award.

Use a separate form for each nominee.

Nomination Procedure: Submit a letter of recommendation not to exceed two pages. Please provide evidence of the following criteria in your letter:

• Excellence in student advising
• Nomination form signature from Department Chair/supervisor verifying role as undergraduate academic advisor
• Contributions to the improvement of the quality of academic advising either within the unit and/or campus wide
• Demonstration of a concerned attitude toward students
• Mastery of institutional regulations, policies and procedures
• Mastery and utilization of appropriate information and referral sources
• Participation in and support of advisor development programs
• Commitment to accessibility of advisors to students
• Excellence in creating or maintaining advising structures and procedures, using advising technology, communication, or other special skills

Nominations may be supported by as many as four letters of recommendation (a student letter is strongly encouraged) and other supporting material such as résumé or Curriculum Vitae. Save nomination form and any letters in the above order as one .pdf document (label by last name of nominee) and submit on CD (2 copies).
