Hospital Pharmaceutical Care

Course Description:
Advanced clinical pharmaceutical health care experience in a hospital
pharmacy setting.
Cr. 6. (0-18)
Prerequisite:
Fourth year standing in the College of Pharmacy or consent of
the instructor.
Course Proficiencies:
The student will be able to:
1. Clarify medication orders after reviewing
for completeness, accuracy, allergies, legality, dose, route,
and frequency.
2. Review medication orders for drug-drug; drug-food; drug-disease
interactions and therapeutic duplication.
3. Review a patient’s medication profile for appropriateness of
therapy, and potential adverse side effects
3. Maintain and update patient profiles with respect to medication
therapy.
4. Participate in the interdisciplinary management of a patient’s
care.
5. Describe federal and state laws for Class C Institutional pharmacies
and controlled substances and how the hospital’s policy and procedures
comply with those laws.
6. Demonstrate knowledge of the different types of formularies
in hospitals and how the Pharmacy and Therapeutics Committee (P&T)
functions to develop a formulary.
7. Develop a monograph for a drug being considered for addition
to the formulary.
8. Perform a drug utilization evaluation.
9. Counsel a patient or caregiver on medications prior to discharge.
10. Evaluate laboratory data with respect to the patient’s disease
state and medications.
11. Monitor drug therapy outcomes.
12. Provide an in-service to pharmacists and other health care
member regarding new drugs and current trends in health-systems
pharmacy.
13. Provide verbal and written drug information to patients or
health care professionals.
14. Describe the role and impact on hospital pharmacy practice
of regulatory agencies such as the FDA, DEA, Texas State Board
of Pharmacy, JCAHO, OBRA, Medicare, OSHA, and DPS.
15. Demonstrate proficiency in communicating with other health
care professionals.
16. Participate in patient rounds with the medical team.
17. Develop and implement a disease state management protocol
(i.e., for hypertension, diabetes, anticoagulation).
18. Recommend appropriate drug therapy and alternative therapeutic
solutions.
19. Identify adverse drug reactions and describe reporting system
for adverse drug reactions within the institution.
