This form allows you to get assistance from the Information Technology Support Center. After you submit your support request, a case number will be emailed to you for future reference. If you cannot receive email, you can still use this form to open your support request. An IT Support Center consultant will contact you by phone within one working day.

If your problem is urgent, please call the IT Support Center at 713-743-1411. The IT Support Center operates 24 hours a day, 7 days a week (except university holidays).
