Section 1: Requesting Service
The purpose of the Service
Request is to provide uniformity and control in the management
and tracking of service requested from the Management Information
Systems (MIS) department.
- Policy Objective
The Service Request policy is intended to:
- Provide clear guidelines for requesting service
from the Management Information Systems (MIS)
department by the University of Houston (UH) user
community.
- Provide a formal yet simple means of authorizing
requests by the user and, reviewing and
approving/disapproving request by MIS.
- Establish traditional audit trails and controls.
- Scope
All services provided by MIS will follow this policy.
Although an official Service Request form is provided
(see exhibit 5.1-A), other written forms (such as
letters, internal memoranda, and email) are acceptable as
long as they contain the needed information and the user
authorization.
