Section 1: Requesting Service

The purpose of the Service Request is to provide uniformity and control in the management and tracking of service requested from the Management Information Systems (MIS) department.

    All services provided by MIS will follow this policy. Although an official Service Request form is provided (see exhibit 5.1-A), other written forms (such as letters, internal memoranda, and email) are acceptable as long as they contain the needed information and the user authorization.
