To apply for financial aid at UH, start by submitting the FAFSA. Here are a few more forms you might need:

Academic Scholarship Appeal: 2013-2014
Students may initiate a scholarship appeal by completing this form if the following reasons apply: the student has been denied/canceled scholarship assistance due to a lack of Satisfactory Academic Progress (SAP) or is requesting special circumstances outside of the Freshman Scholarship Criteria (such as a request to take less than 12 or study abroad). Complete this form and provide the necessary documentation to support the reason(s) for appealing.
Academic Scholarship Petition: 2013-2014
Students may initiate a scholarship petition by completing this form if the following reasons apply: student has prior knowledge of circumstances that will prohibit them from meeting the scholarship criteria, student is requesting to participate in a COOP or other sanctioned university program or student would like to request scholarship funds be applied to a semester outside of the traditional award terms (Fall/Spring). If one or more of the previously stated circumstances apply, please complete this form and provide the necessary documentation to support the reason(s) for petitioning. This form is to be used for scholarships only.
Alternative Loan Budget: 2013-2014
Required from students who are only applying for an alternative loan and are declining all other financial aid. The data collected is used to calculate the student’s cost of education. If a FASFA has been completed, please do not submit this form.
Be-On-Time (BOT) Forgiveness Eligibility Confirmation Form
The purpose of the Texas Be-On-Time Loan program is to provide eligible Texas students no-interest loans to attend colleges and universities in Texas. If a student meets specified goals, the entire loan amount can be forgiven. Upon graduation, the student will need to fill out the BOT Forgiveness Eligibility Confirmation Form.
Child Support Verification Worksheet
This form is requested from students whose Free Application for Federal Student Aid (FAFSA) was Federally selected for review in a process called, "Verification." You or a member(s) of your household paid child support in 2012, as part of the verification process, you must confirm and provide proof of this benefit. Complete this worksheet and provide the appropriate documentation only if requested or "initiated" in your myUH To Do List.

Family Educational Rights and Privacy Act (FERPA)
The Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 CFR Part 99) is a Federal law that protects the privacy of student education records. The law applies to all schools that receive funds under an applicable program of the U.S. Department of Education. FERPA gives parents certain rights with respect to their children's education records. Schools must have written permission from the student in order to release any information from a student's education record.A student must complete the FERPA form and turn it in to the Office of Scholarships and Financial Aid in order for our office to speak about the student’s account with whom permission has been given. If the form is being faxed or mailed in, a copy of the student’s ID or driver’s license must be attached.

2013-2014 Food Stamps & Child Support Verification Worksheet
This form is requested from students whose Free Application for Federal Student Aid (FAFSA) was Federally selected for review in a process called "Verification." You or a member(s) of your household received Food Stamps or participated in the Supplemental Nutrition Assistance Program (SNAP) in 2011 and/or 2012 OR paid child support in 2012. As part of the verification process, you must confirm and provide proof of this benefit. Complete this worksheet and provide the appropriate documentation only if requested or "initiated" in your myUH To Do List.

2014-2015 Food Stamps & Child Support Verification Worksheet

2013-2014 Food Stamps Verification

GRE Fee Reduction Certificate: 2013-2014
Individuals who can be granted a GRE fee waiver for 2013-2014 (for tests from July 1, 2013 to June 30, 2014) are seniors receiving financial aid who have a current FAFSA on file with UH. Educational Testing Service offers a limited number of GRE Fee Reduction Vouchers on a first-come, first-served, basis that are available for college seniors and college graduates who meet eligibility requirements. A Fee Reduction Voucher may be used for one General Test and/or one Subject Test. Eligibility for participation in the Fee Reduction Program is limited to one time only. Thereafter, the full test fee will be charged for additional tests for which the candidate registers. Individuals meeting the eligibility requirements will be required to pay 50 percent of the full test fee.

High School Completion Status: 2013-2014
This form is requested from certain applicants whose Free Application for Federal Student Aid (FAFSA) was federally selected for verification of their high school completion status. The form and acceptable documentation should be submitted only if it is "initiated" on your To Do List in your myUH self-service account.

Independent Verification Worksheet: 2014-2015

Institutional Grants Eligibility Review Form
This form is to review students for possible institutional grant eligibility due to meeting or exceeding their aggregate limits for federal aid. Students with institutional grants on hold or canceled due to meeting or exceeding federal aggregate limits can complete this form.

Parent 2012 Tax Nonfiler Form
This form is requested from dependent students to certify that his/her parent(s) has not and is not required to file an income tax return. The form should be submitted only if it is "initiated" in your myUH To Do List.

Parent 2013 Tax Nonfiler Form
This form is requested from dependent students to certify that his/her parent(s) has not and is not required to file an income tax return. The form should be submitted only if it is "initiated" in your myUH To Do List.

Perkins Loan E-sign Promissory Note
This must be completed to receive Perkins funds. The signature is good for 10 years. This promissory note is NOT for Federal Stafford Loans.

Perkins Loan Entrance Counseling
This must be completed in order to receive Perkins funds. Prior to disbursing Federal Perkins Loan funds, regulations require that an entrance counseling session must be completed by the borrower. The counseling session helps guide the borrower in managing their student loans, both during and after college. This entrance counseling session is NOT for Federal Stafford Loans.

Perkins Loan Exit Interview
This is completed when a student leaves or graduates from the University or is no longer at least half-time enrollment status. This does not include the summer semester. Students do not have to enroll in summer as long as they resume classes the following fall semester.

Post-Baccalaureate Certification Statement: 2013-2014
This form is requested from all students admitted in a post‐baccalaureate program. If you are not in a graduate program, but have a bachelor's degree, you are considered a post‐baccalaureate student for enrollment purposes. Post‐baccalaureate students are not eligible to receive federal or state grants. They may be eligible to receive federal loan assistance, pending aggregate and program eligibility. Post‐baccalaureate students must be enrolled in at least 6 hours to receive loans. This form is required to determine aid eligibility and should be submitted only if it is "initiated" on your To Do List in your myUH self‐service account.

Private Loan Self-Certification
This form is required from all admitted or enrolled students pursuing a private educational loan. Section 2 of this form will be completed by the Office of Scholarships and Financial Aid (SFA) upon request. The applicant must submit the completed form to his or her lender. UH SFA does not process or track this form. Some lenders may have this requirement included in their private loan application process. The lender cannot submit funds to UH until the form is received.Pursuant to Section 155 of the Higher Education Act of 1965, as amended, (HEA) and to satisfy the requirements of Section 128(e)(3) of the Truth in Lending Act, a lender must obtain a self-certification signed by the applicant before disbursing a private education loan. The school is required on request to provide this form or the required information only for students admitted or enrolled at the school. Throughout this Applicant Self-Certification, “you” and “your” refer to the applicant who is applying for the loan. The applicant and the student may be the same person.

Proof of Course Completion Form: Fall 2013
If a student who began attendance during Fall 2013 has not officially withdrawn and fails to earn a passing grade in at least one course offered over an entire period, the institution must assume, for Title IV purposes, that the student has unofficially withdrawn; unless the institution can document that the student completed the period (DCL GEN-04-03, February 2004). Students who are subject to a Return of Title IV Funds calculation due to their unofficial withdrawal from all courses must submit this form. Revision Request: 2013-2014
This form is used to reinstate funds, to cancel funds, or to make adjustments to your financial aid package based on a change in eligibility.

This form is used to reinstate funds, to cancel funds, or to make adjustments to your financial aid package based on a change in eligibility.Satisfactory Academic Progress (SAP) Appeal: Summer 2013The deadline to submit an appeal for Summer 2013 is the official census date of your latest summer enrollment. For Summer Sessions 1, 2 and 3, the deadline was June 6, 2013. For Summer Session 4, the deadline was July 11, 2013.

Satisfactory Academic Progress (SAP) Appeal: 2013-2014
Satisfactory Academic Progress will be monitored once a year. Returning students who may not be meeting SAP standards are encouraged to submit their 2013-2014 FAFSA before August 26, 2013, so that they may receive their SAP status before the aid year starts. Satisfactory Academic Progress will be monitored once a year for 2013-2014 and runs in the fall semester. The Fall 2013 deadline for submitting appeals has now passed, but Spring-only students may submit the Spring 2014 SAP appeals forms if they did not previously submit an appeal for Fall 2013. If you have already submitted a Fall 2013 SAP appeal but were denied, you may NOT submit a Spring 2014 SAP appeal.

Special Circumstance Appeal
A student’s eligibility for financial aid is calculated based on the information provided on the Free Application for Federal Student Aid (FAFSA). However, UH recognizes that many families experience changes in income, or have family situations that are not reflected on the FAFSA. This form provides information regarding the types of unusual or extenuating circumstances that can be considered; it is also a guide to understand the process of submitting a Special Circumstance Appeal.

Summer aid eligibility for Federal funds is determined by any remaining funds that were not utilized during the fall and spring semesters.

2013-2014 TEACH Grant Application
This form is for students interested in teaching in a public or private elementary or secondary school that serves students from low-income families (valid until June 6, 2014).

Texas Armed Services Scholarship Program Eligibility Review Request: 2013-2014
This form is requested for any student receiving the Texas Armed Services Scholarship Program (TASSP) award. The information on this form is used to determine students' eligibility for the scholarship.
Texas Application for State Financial Aid (TASFA): 2014-2015
Deadline: March 15
Please note that availability of funds is very limited. Submitting an application does not guarantee eligibility.
Students qualifying for state residency under House Bill 1403/Senate Bill 1528 are eligible to apply for state financial aid using the TASFA. Texas Grant Eligibility Review Request Form: 2013-2014

Texas Grant Hardship Provision Request Form: 2013-2014
This form is for students who were ineligible for continuing in the Texas Grant program due to not meeting the academic requirements. This is not an appeal but a request for reconsideration due to a documented hardship.

Top Ten Percent Scholarship Eligibility Review Request: 2013-2014
Deadline: Not currently accepting forms
The purpose of this application is to review previous recipients of the Top Ten Percent Scholarship for eligibility. If you are a freshman or have not previously received this award your application will be voided.

Transfer Monitor: Spring 2014 
If you are a spring transfer student, we are required to review your student aid history within the National Student Loan Data System (NSLDS). If you have active federal student aid at another institution within the same academic year in which you are currently enrolled at our institution, you must cancel your pending spring aid at your previous institution and complete and submit this form. Please refer to our Spring Aid website for more information.

Transfer Monitor: Summer 2013
This form is required from any student who has a Transfer Monitor Hold on their account or has transferred to UH, but received federal student aid within the same academic year at another institution, or if they experienced a discontinuation of enrollment during the 2012-2013 school year. The information provided on this form allows the Office of Scholarships and Financial Aid to accurately package the student’s financial aid. This form must first be completed by the student’s previous school’s Financial Aid Office, then returned to the Office of Scholarships and Financial Aid at UH.

Unusual Enrollment History Form: 2013-2014
Unusual Enrollment History Form: 2014-2015

This form is to be completed by students who have been flagged for "unusual enrollment history" by the U.S. Department of Education. A student may be flagged as a result of having received federal Pell Grants at multiple institutions in recent years. The University of Houston's Financial Aid Department will determine if a student had a valid reason for having unusual enrollment at multiple institutions.
