To apply for financial aid at UH, start by submitting the FAFSA. Here are a few more forms you might need:

Academic Scholarship Appeal: 2007-2008 Version (PDF)
Students may initiate a Scholarship appeal by completing this form if the following reasons apply: the student has been denied/cancelled scholarship assistance due to a lack of satisfactory academic progress or you are requesting special circumstances outside of the Freshman Scholarship Criteria (such as a request to take less than 12 hours or study abroad). Complete this form and provide the necessary documentation to support the reason(s) for appealing.

(Deadline to submit a Scholarship Appeal is the 20th Day of Class for each Semester)
Alternative Loan Budget Form (PDF)
Required for students who are only applying for an alternative loan and are declining all other financial aid. The data collected is used to calculate your cost of education. If you have completed a 2008-2009 FAFSA, please do not submit this form.
Asset/Business Questionnaire: 2007-2008 Version (PDF)

This form is used to resolve any conflicts that may exist between your tax return income information and FAFSA asset and business values.
Ensign Promissory Note
This must be completed to receive Perkins funds. The signature is good for 10 years.
Entrance Interview
This must be completed to receive Perkins funds.
Exit Interview
This is completed when the student leaves or graduates from the university or is no longer at least half time status. This does not include summer sessions. Students do not have to enroll in summer sessions as long as they resumes classes the following fall.

Required for all parents borrowing a Federal PLUS Loan.

Required of all students who are enrolled in post-baccalaureate programs.
Revision Request: 2007-2008 Version (PDF)
This form is used to reinstate funds, to cancel funds, or to make adjustments to your financial aid package based on a change in eligibility.
Satisfactory Academic Progress Appeal Application: 2007-2008 Version (PDF)
Satisfactory Academic Progress Appeal Application: 2008-2009 Version (PDF)
This form is used when a student's grades or academic progress slips due to extenuating circumstances. The student can use this form to initiate an appeal for continuation of funds during a probationary period.

Your eligibility for financial aid was originally calculated based on the information you provided on the Free Application for Federal Student Aid (FAFSA). However, we recognize that many families experience changes in income, or have family situations that are not reflected on the FAFSA.This form provides information regarding the types of unusual or extenuating circumstances that can be considered, and guides you through the process of submitting a Special Circumstance Appeal.
Study Abroad
Required for students who are seeking financial aid and are enrolled in a UH Study Abroad program.
Summer Data Form for 2008 (PDF)
Required for all summer Financial Aid applicants. Please read the instructions (doc) for more information, due June 13, 2008.
Supplemental Loan Request Form
Required if you are using a lender that does not guarantee their loans through Texas Guaranteed (TG).
