Campus Leaders Reception Nomination Form

Outstanding Leadership

This award is presented to a student that has contributed to his/her organization(s) by serving as role models of excellence to other members. This student has been dedicated and shown outstanding efforts to make a difference in his/her organization(s) as well as in other areas of campus life.

Nomination Requirements
  1. Must not be the president of a registered student organization
  2. Completed nomination form
  3. Résumé from the candidate denoting leadership positions, membership in registered student organizations, community service, and/or work experience. Email résumé to salo@central.uh.edu
  4. A letter of recommendation (no more than 5000 characters) describing why the nominee should be considered for the award including the following information:
    • Description of organizational responsibilities that the nominee has held at the University of Houston
    • Description of how the nominee has empowered other students in his/her commitment to involvement and service
    • Description of the specific contributions that the nominee has made to co-curricular life at the University of Houston

Deadline to submit the nomination form, letter of recommendation, and additional documentation is 5:00 PM on Friday, March 8th, 2013. Nominated individuals and student organizations will be invited to attend the Campus Leaders Reception on Tuesday, April 23rd, 2013 at 5:30 PM in the University Center Houston Room.
