Instructions for Request for Individualized Testing Accommodations (RITA)
Student Responsibilities:
- Informing instructors that they have a documented disability as early in the semester as possible.
- Submitting to their instructors copies of their Student Accommodation Form (SAF), Academic Accommodations Evaluation Committee (AAEC) approval, and RITA form.
- Completing Section 1 of the RITA form.
- Returning the white copy of each completed RITA form to CSD at least 24 hours before the first exam.
- Informing instructors of any schedule conflicts with exam start dates/times, and arranging for alternate dates, if possible.
CSD Responsibilities:
- Assuring test security, including exam pick-up and delivery, exam maintenance, and proctoring.
- Returning exams to instructors within two (2) business days of exam completion.
- Adhering only to recommended exam accommodations.
- Exams are the property of the instructor. CSD will not change the date or time of an exam without the instructors permission to do so.
Instructor Responsibilities:
- Completing Section 2 of the RITA form. Please list as many exam dates as possible.
- Indicating in the space marked “Time,” the time the student may take the exam, not the time of the class. All exams taken at CSD must be scheduled within CSD’s hours of operation as shown below.
- If the exam dates have not been scheduled when the RITA form is filled out, please complete a FAX RITA Form upon scheduling an exam, and fax it to CSD at (713) 743-5396. We will then put the student on the schedule and arrange to pick up the exam prior to the exam date.
- If you plan to deliver and/or pick up the exam, please fill in the date and time in the “Options” section.
- Completing the “Special Instructions” section, and indicating whether or not the student may take unproctored breaks during the exam.
- Signing the RITA form and returning it to the student as soon as possible. Keep the pink copy and forward the gold copy to your department contact.
CSD hours of operation are 8:00 to 5:00, Monday to Friday, and 8:00 to 7:00, Mondays and Tuesdays during the Fall and Spring semesters.
