Documentation: Psychological Disorders

- Diagnostic Information: A diagnosis of a psychological disorder, including a description of how the diagnosis was determined following DSM-IV codes must be indicated. Information obtained through clinical and diagnostic interviews should be included.
- Supporting Documentation: A battery of personality testing may be included as part of a comprehensive psychological evaluation. One test or assessment battery alone does not constitute adequate documentation. Evaluations should have been conducted within six (6) months prior to the request for academic accommodations. The University has the right to request updated evaluations after accommodations have been recommended.
- Treatment: Information regarding the student's current treatment should include whether psychotherapy and/or psychotropic medication (type/dosage/side effects) have been or promise to be effective in alleviating symptoms, and whether they may interfere with a student's academic functioning.
- Functional Limitations: Documentation should indicate any functional limitations caused by the disorder and whether the limitations are temporary or long-standing.
- Accommodation Recommendations: Specific recommendations for academic accommodations for the functional limitations evaluated should be included.
- Evaluator Qualifications: Documentation should include a thorough clinical and diagnostic interview conducted by a licensed psychiatrist, licensed clinical or counseling psychologist, or licensed social worker on his/her letterhead, and a DSM-IV diagnosis including diagnostic code(s).
