Jacob Zalewski Scholarship - 2013-2014

A new scholarship opportunity is available to eligible University of Houston students who are registered with the Center for Students with DisABILITIES for the 2013-2014 academic year!

The Jacob Zalewski Scholarship Fund is a gift from UH alumnus Jacob Zalewski. Scholarship recipients will be awarded up to $1,000 for the 2013-2014 academic year.

Jacob graduated from UH in 2008 with a Bachelor's degree in Communications. He was a scholarship recipient and fortunate to have opportunities and supports that helped him to succeed in college. Jacob wanted to assist other students in pursuing their educational dreams at UH, and raised funds for these scholarships through his non-profit organization, the One Step Closer Foundation.

Applicant Eligibility Requirements

1. Be a UH student who is registered with CSD and has a documented physical
   or health impairment as dictated in the Center's guidelines.
2. Be eligible for Financial Aid for the 2013-2014 academic year.
3. Be a full-time or full-time equivalent, degree-seeking student.
4. Maintain a GPA of 2.0 or better, with preference given to students with
   GPA's of 2.5 or better.

Selection Criteria

The Center for Students with DisABILITIES Scholarship Selection Committee will select the scholarship recipients based on merit and financial need. Recipient selection will be based upon information received from the Scholarships and Financial Aid Office, transcripts, evaluation forms/letters of recommendation, and the applicant's essay. All decisions of the Committee are final.

Applicants need to submit all of the following in order to be considered for a scholarship no later than June 28, 2013:

1. Completed Personal Information Form .doc
2. Copy of current course schedule and college transcripts (unofficial is acceptable).
3. A minimum of two (2) completed Recommendation Forms and letters of recommendation. Letters should be printed on official letterhead. One letter should be from a current or previous instructor from student's major field of study. One letter can be from another college professor, high school teacher or a workplace supervisor.
4. An essay including the following:
   • Your major and academic goals
   • Your career objectives
   • How this scholarship would assist you in achieving your academic goals.
   • Discuss strategies you have used to compensate for your disability in academic and/or vocational settings.
5. Submit a complete original application packet plus six (6) copies to CSD on or before June 28, 2013.

Awards

Awards will become effective in the fall 2013 semester, in an amount not to exceed $1,000 per recipient. Funds will be automatically deposited in recipient's account, to be applied toward tuition, room, board, books and other academic-related expenses. This is a single year award. Awards are typically split between the fall and spring semesters.
