The following Manuals of Administrative Policies and Procedures were approved by President Khator as official university policy during Quarter 2, Fiscal Year 2017 (December, 2016 and January and February, 2017):

MAPP 02.01.01 - Pay Guidelines for Staff Employees December 7, 2016 Removed Section V.D.1.i regarding hiring college or division having full authority to set promotional increases. Also removed Section V.D.4 on retention bonuses.
MAPP 02.01.01 was in the Quarter 1, FY 2017 Review Cycle.
MAPP 02.02.01 - Family and Medical Leave December 2, 2016 Added FML leave commencement to definition of child in Section III.A. Revised definition for Intermittent leave or reduced leave schedule in Section III.I. Included worker's compensation benefits for accrued paid leave in Section IV.C. Updated Section V.C with eligibility notice and rights and responsibility notice as part of FMLA application on HR web site. Revised Section VI.F to place employee on paid or unpaid leave in University's HR/Payroll system by removing ePAR process; this update also took place in
Section VII.A.1.a upon approval of FML. Removed ePAR process for active employee status in
Section VIII.B.

MAPP 02.02.01 was in the Quarter 1, FY 2017 Review Cycle.
MAPP 02.02.06 - Workers' Compensation February 23, 2017 Made corrections to links and titles as applicable. Added references to Section VIII. Modified Accident Forms, Employee's Election Regarding Utilization of Sick and Annual Leave, Employer's Wage Statement, non-disabling illness/injury, and Supplemental Report of Injury in Section III. Made minor redlines in Section V.A.2. Added complete process for reporting exposure to HIV in
Section V.A.3. Added requirement for receiving completed SORM-80 form no later than 5 calendar days after the first full day of lost time in
Section V.A.5.a. Added current process for accident-related lost time PASS documentation in
Section V.B.5.d.

MAPP 02.02.06 was in the Quarter 2, FY 2017 Review Cycle.
MAPP 02.03.05 - Background Checks for Employees February 23, 2017 Changed title from Security Sensitive Positions to Background Checks For Employees, and rewrote process to emphasize criminal background checks. New requirement for all positions to receive a background check, and process to be done by use of a vendor instead of the previous UHDPS process. The responsibility is assigned to Human Resources and the Office of Faculty Affairs for completion, retention, and destruction of obtained documents. Added information to Section VII.B for an evaluation to be completed if a candidate is recommended for employment in spite of a positive background check. Added Section XIII for references.
MAPP 02.03.05 was in the Quarter 2, FY 2017 Review Cycle.

MAPP 02.06.02 - Staff Tuition Scholarship Program February 23, 2017 Removed Section III.A on College Release Time; courses taken for this program must involve a degree program at University of Houston. Removed information on career paths and Grade Point Average in Section III. Revised Section III.C to indicate taking classes at other universities, including other system components, will not be approved. Added Grade Point Average information in Section IV.F. Removed Section V.A on credit courses. Revised total for maximum scholarship amount from $1,200 to $2,000. Removed composition of Staff Tuition Scholarship Committee in Section V.B, and added task to review applications and select award recipients. Revised links and titles as applicable.
MAPP 02.06.02 was in the Quarter 2, FY 2017 Review Cycle.

MAPP 04.01.03 - Vouchers February 23, 2017 Updated titles and links as applicable. Updated Section VII.K to emphasize local, non-federal funds, and indicate the President may be reimbursed for more than 20% gratuity based on actual expenditures.
MAPP 04.01.03 was in the Quarter 2, FY 2017 Review Cycle.
MAPP 04.04.01A - Contracting - General February 23, 2017 Added information to Section II.C on delegation of authority being made in writing and filed with the OCA by September 1st of each fiscal year. Made modification to Section V.J regarding the Office of Risk Management needing COI unless a waiver is granted. Updated titles and links as applicable. Added Section IX, References.
MAPP 04.04.01A was in the Quarter 2, FY 2017 Review Cycle.
MAPP 05.02.02 - Official Functions and Discretionary Expenditures February 23, 2017 Updated titles and links as applicable. Added Section III.G definition for gratuity, indicating that President may be reimbursed for more than 20% gratuity based on actual expenditures. Added information to Section VII.A, indicating business meals involving the President will be paid or reimbursed based on actual expenses, including gratuity, supported by receipts.
MAPP 05.02.02 was in the Quarter 2, FY 2017 Review Cycle.
MAPP 06.01.01 - Emergency Management Policy February 23, 2017 Applied revised MAPP template and added new Revision Log. Changed title from Emergency Management Plan to Emergency Management Policy. Revised entire text to reflect current operating requirements. Deleted all Addendums and Index Terms.
MAPP 06.01.01 was in the Quarter 2, FY 2017 Review Cycle.
MAPP 06.05.03 - Motor Vehicle Record Evaluation February 23, 2017 Revised Motor Vehicle Request form and updated link. Updated titles and links as appropriate. No additional redlines were indicated by the Subject Matter Expert (SME).
MAPP 06.05.03 was in the Quarter 2, FY 2017 Review Cycle.
MAPP 08.01.01 - Sponsored Research Activities February 10, 2017 Added Section II.F on Principal Investigator responsibilities for regulatory compliance approvals. Added link to Research Administration Certification Program (RACP) in Section II.C. Updated responsible party's title.
MAPP 08.01.01 was in the Quarter 2, FY 2017 Review Cycle.
MAPP 09.05.01 - Building Coordinator Program

December 20, 2016 The purpose of this document is to outline the purpose and scope of the Building Coordinator Program, the program's benefit to the University community, and the role and expectations of the staff members who serve as Building Coordinators for their facilities.
New MAPP 09.05.01 was in the Quarter 1, FY 2017 Review Cycle.
MAPP 09.06.01 - Key Control Policy (New MAPP) February 23, 2017 This document establishes the guidelines for the campus key control process which is designed to help protect the life, property, and security of each facility and its occupants and users. The Key Control Policy establishes a recorded chain of accountability and access for all credentials, key holders, and locations.
New MAPP 09.06.01 was in the Quarter 2, FY 2017 Review Cycle.
MAPP 11.03.03 - Non-Resident Tuition Employment Waivers December 20, 2016 Applied revised MAPP template and added new Revision Log. Changed section from 05.03.02 to 11.03.03 to reflect "Tuition and Fees" subject for MAPP. Rewrote entire procedure to reflect current operating requirements and Texas Legislature changes.
MAPP 11.03.03 was in the Quarter 1, FY 2017 Review Cycle.
