MAPPs Approved In Quarter 2, Fiscal Year 2013

The following Manuals of Administrative Policies and Procedures were approved by the President as official university policy for Quarter 2, Fiscal Year 2013 (December 2012 and January/February 2013):

MAPP 01.01.01 - Administrative Policies and Procedures December 5, 2012 Applied revised MAPP template and added new Revision Log. Removed the definition of MAPP in Section II. Replaced the phrase "Business Services" with "University Policies and Procedures Coordinator" throughout the document. Added "Supported Policy Information" and "Revision Log" to formatting information in Section V. Added note on SME responsibilities for documented review period MAPPs in Section VI.B. Added EVP for Administration and Finance review period before the Campus Review in Section VI.C. The Executive Review period is set to 20 working days in Section VI.D.4. Updated the approval process in Section VI.D.7. Added the review period for the University Coordinating Commission (UCC). Added a note on communication of changes after the Executive Review cycle and sending a finalized copy of the MAPP to the UCC. Approved MAPPs are posted to the MAPP web page in five working days as documented in Section VII.B. Added the responsible party of the AVP for Administration. The review period was changed from every three years on or before August 31st to every three years on or before March 1st. Removed Index terms. Added MAPP Review flowchart to Section XII.

MAPP 01.01.02 - University Coordinating Commission December 11, 2012 This new MAPP outlines the membership and responsibilities of the University Coordinating Commission for sending out documentation for review.
New MAPP 01.01.02 was in the Quarter 4, FY 2012 review cycle.
MAPP 02.02.03 - Leaves of Absence December 5, 2012 Applied revised MAPP template and added new Revision Log. Added hyperlinks to documents and areas as applicable. Changed Personnel Action Request (PAR) to Electronic Personnel Action Request (ePAR) throughout the document. In Section V, document was added that leave requests cannot be used across multiple payroll periods. Changed the definition for employee's family in Section VI.F on Bereavement Leave. Added information on Jury Duty and the submittal of the Jury Duty Release notice to supervision upon return from Jury Duty in
Section VI.G. Corrected the reference to the Workers' Compensation MAPP in Section VIII.A, and also corrected the department name. In Section VIII.D, a paragraph was added concerning military caregiver leave for a current member of the Armed Forces. Added Section XII, References.

MAPP 02.02.03 was in the Quarter 4, FY 2012 review cycle.

MAPP 02.03.06 - Employment Eligibility Verification Form I-9 December 11, 2012 This new MAPP provides general guidelines to ensure compliance with completing the Employment Eligibility Verification Form I-9.

MAPP 02.04.06 - Reduction in Force December 5, 2012 Applied revised MAPP template and added new Revision Log. Changed Affirmative Action to Equal Employment Opportunity throughout text to reflect current organizational structure. Redefined "regular employee" in Section II to emphasize nonfaculty positions of at least 20 hours a week, excluding student status. Removed information concerning specific plan review in Section V.B. Added General Counsel to the administrative channels in Section V.C. Removed the reason to bypass RIF employees that did not work in the administrative unit looking for personnel in Section VII.C.2. Redlined Section VIII, Appeal Process. Added SAM 01.D.07 to Section XI, References. Provided minor redlines to RIF Checklist (Addendum A).
MAPP 02.04.06 was in the Quarter 4, FY 2012 review cycle.

MAPP 02.05.01 - Access To/Maintenance of Personnel Records January 17, 2013 Applied revised MAPP template and added new Revision Log. Added links to all documents and web sites throughout the procedure. Added Student Employee to Section II.B. Removed beneficiary insurance coverage from personnel file maintenance in Section III.A.2; other changes are to be made through employee self service or sent in writing. Removed Section III.B.4 on each department maintaining personnel files. If an employee or designated representation requests a copy of any documentation, they may be required to pay a copying charge. In Section IV.B.2, all requests for student information require prior authorization for release via FERPA. Added information on social security numbers to Section IV.C.1. Emergency contact information can be excluded from public access. Changed the responsible party to the AVP for Finance. Changed the title for the EVP for Administration and Finance for approval. Added FERPA to Section VII, References. Removed Index Terms.
MAPP 02.05.01 was in the Quarter 4, FY 2012 review cycle.

MAPP 02.07.02 - Reasonable Workplace Accommodations for Employees With Disabilities February 15, 2013 Replaced MAPP 02.07.01 with SAM 01.D.07, Discrimination and Harassment Policy, in Sections VII and X. No additional redlines were needed per the Subject Matter Expert.
MAPP 02.07.02 was in the Quarter 2, FY 2013 review cycle as a housekeeping revision.
