Two reporting functions have
been combined into one quick
online form. You must answer
all applicable questions to
receive credit for
completing the form.
The first part of the form
addresses external
consulting and outside
employment activities that
the Chancellor is required
to report to the Board of
Regents annually per
BOR
Policy 57.02.
All full time faculty and
all exempt staff must
acknowledge any external
consulting and outside
employment activities
performed between September
1, 2007 and August 31, 2008.
Please note that unpaid
public service is not
included in this reporting,
nor are occasional lectures
that include fees, unless
these activities require
significant amounts of time
or otherwise conflict with
regular System obligations.

The second reporting
requirement this online form
addresses is the annual
disclosure of related-party
interests. All faculty and
staff in a position to
originate purchase requests
or influence purchasing
decisions shall annually
complete a disclosure
statement regarding
related-party interests.
