Performance Communication &
Development Checklist (Section A)
This form ensures the staff member has seen the overall review
being submitted to the official personnel record. It is
completed at the end of the performance period. The supervisor
and staff member check which performance communication and
development method(s) were used and check whether the staff
member met, exceeded, or did not meet performance expectations.
Then the form is signed and forwarded to Human Resources.

Summary Notes of Performance Informal
Conversations Between Supervisor & Staff (Section B)
This option is designed to summarize multiple informal
performance-focused dialogues between the supervisor and staff
member. Once summarized, both supervisor and staff member
initial the dated entry. This form is kept in the department
file.

Performance Communication Discussion
Guide (Section C)
This is a supplemental page to Summary Notes of Performance
Informal Conversations Between Supervisor & Staff. This option
is designed to guide and document an annual performance
discussion between the supervisor and staff member. Summary
notes of the discussion are documented and both supervisor and
staff member initial the completed form. This section must be
attached for “Exceeds Expectations” rating.

D)
This option is designed to document a performance development
plan between the supervisor and staff member. Both initial the
plan. Summaries documenting progress can be added. This section
must be attached for “Needs Improvement” rating.

Performance by Initiative Plan (Section E)
This option documents a staff member's work alignment and
expected outcomes to organizational and departmental
initiatives. This form also documents work activities,
relationships, anticipated outcomes and performance standards.
Summaries documenting progress can be added. This form is kept
in the department file and by the staff member.
